In this article you will learn what a Public deed of identification is, where it will be issued to you and which documents you need in order to have it issued.
What is a Public deed of identification?
A Public deed of identification is a document evidencing that you have been identified. At your request the deed is issued and certified by an official or a public administration contact point.
You then send the issued and certified deed to our correspondence address by post.
Where will the Public deed be issued to you?
The Public deed will be issued to you by:
public administration contact points – CzechPoint
a regional authority or the municipal authority of a municipality with extended powers
a notary
The easiest way is to go to a CzechPoint, which you will find at a Czech Post office, for example. A list of all CzechPoint locations is available on the official website.
Going to an authority is somewhat less convenient, mainly because of the less accessible office hours. You will have to make an appointment for a specific time with a notary, and you should also expect a higher fee for drawing up the deed.
What does the Public deed of identification contain?
The Public deed of identification must contain:
who (the official), at whose request (yours) and for what purpose* carried out the identification,
the identification details of the person being identified (you),
certification of your declaration of identification
the place and date the deed was drawn up, or the place and date where and when the identification took place, if the place or date of drawing up differs
the signature of the person who carried out the identification, the imprint of their official stamp and the serial number in the register of deeds of identification.
💡 GoPay tip: As the purpose* of the identification we recommend stating the following text: Fulfilment of the identification obligation arising from Section 7 of Act No. 253/2008 Coll.
What do you need to take with you when you want to request identification?
an identity document
an extract from the commercial register
a certified power of attorney, if you want an authorised representative to be identified
You should not bring the Public deed with you already filled in – the relevant authority will issue it to you.
The official carries out the identification on the basis of your identity documents (ID card, driving licence or passport). Copies of these documents must be attached to the Public deed of identification and must be legible.
If an authorised representative wants to be identified, a certified copy of the power of attorney must also be attached to the Public deed.
The official then firmly binds the Public deed together with all its attachments into one file.
💡 GoPay tip: Watch out for common mistakes
1. The Public deed must be complete and must be issued by an official
A common mistake is that we receive a document with a certified signature but the identification is missing. Please avoid this mistake and send us a complete Public deed of identification.
2. State GoPay Czech odštěpný závod WORLDLINE NV as the obliged entity
Do not state your own company as the obliged entity.
3. The person who signed the contract with us must be identified
The identification must be undergone by the person who signed the registration form, i.e. the contract, with us.
